Before using the gym you are advised to complete an induction and must complete a Health Questionnaire. In the Health Questionnaire you must declare that to the best of your knowledge the information provided to us is complete and accurate.
Although we advise an induction, you are able to opt out if you have used a gym before, are over 16 and have no health concerns. If you answer yes to any of the following questions you may need to seek the advice of your doctor. If you do not seek this advice, you may not be able to use the gym. The list below is not exhaustive and the Fitness Instructor may make the decision to refer you to your GP or our Exercise Referral team at their discretion.
1. Have you had asthma as an adult?
2. Do you have arthritis?
3. Do you have diabetes?
4. Do you have epilepsy?
5. Do you have high or low blood pressure?
6. Do you have a heart condition?
7. Do you lose consciousness or balance due to dizziness?
8. Do you have a muscular, joint or back disorder?
9. Have you been pregnant/given birth within the last 6 months?
10. Did you have any operations or injury in the last year?
11. Do you have any other conditions that will affect your ability to exercise?
You are responsible for monitoring your own condition throughout any exercise sessions (including group classes) and should cease participation should any unusual symptoms occur. You should also notify the Fitness Team of any changes to the above answers before continuing to exercise.
